Accident prevention policy

1. Statement

In order to maintain a healthy work-force and a safe environment for our employees, students, visitors, etc we aim to:

  • Identify the causes of accidents and the conditions under which they are most likely to occur
  • Carry out regular risk assessments, audits, inspections and checks to minimise the risk of accidents occurring
  • Encourage you to report accidents or near misses so that we can learn from events
  • Investigate all accidents to establish their causes to prevent a recurrence
  • Maintain good records and statistics to identify any problem areas or unsatisfactory trends

2. Organisation and responsibilities

2.1 All workers (staff, students and visiting workers)

Your responsibilities include: 

  • Being aware of and observing the local accident procedure
  • Reporting and documenting all accidents, incidents and observed hazards
  • Co-operating with your health and safety advisor in relation to any investigation
  • Ensuring visitors are aware of our Health and Safety procedures

2.2 Institute Directors

You are responsible for ensuring the institute has a health and safety management system capable of:

  • Reducing accidents and injury by providing a safe and healthy work environment at your institute
  • Ensuring that local procedures are developed and are regularly reviewed
  • Taking all practicable steps to ensure that everyone who works or visits your institute is aware of the accident reporting system
  • Ensuring that the institute has appropriate first aid and emergency care (or other assistance) where required
  • Regularly evaluating accident statistics with senior management 
  • Checking that mishaps falling within the scope of the RIDDOR Regulations are reported to HSE (para. 7 refers) 
  • Ensuring that all accidents and incidents are appropriately investigated and that recommendations are acted on
  • Co-operating with any external investigation by the enforcement authorities

2.3 Visitors and contractors

Anyone who is involved in an accident or near-miss incident whilst on our premises or travelling on our behalf must report the matter immediately. They should also notify their own employer where applicable.

3. Arrangements

3.1 Reporting incidents

We do not operate a “threshold limit” on the type or severity of accident or near miss that should be reported and recorded. There is a known link between the number of incidents or near miss occurrences, and the number of accidents that occur. It is therefore important that if you are involved in an accident or near miss which happens at work, or during an activity that is on our behalf, however minor or apparently trivial, you should report it as soon as possible. By doing so, you might prevent the possibility of a future accident.

3.2 Accident investigation

All accidents will be reviewed by the Health and Safety Advisor, to see in they need to be investigated. The investigation will be carried out by the Health and Safety Advisor or a suitable person whom they have nominated depending on the nature and seriousness of the accident. The type and level of the investigation will be appropriate to the circumstances and severity or potential severity of the accident. In all cases Health and Safety Advisers will be provided with the facts about the accident, including the accident location, the time and date of the accident, the work activity being undertaken, the people involved and the sequence of events.

It may also be necessary for the accident investigator to:

  • Inspect the accident site once the area has been made safe, and take appropriate photographs
  • Interview and obtain written statements from all relevant parties
  • Obtain physical evidence (equipment, damaged products, etc)
  • Review relevant documentation (training, inspection, maintenance records, work procedures, etc)
  • Review existing relevant risk assessments and corresponding arrangements.

Analysis of this information will help identify the cause of an accident. The investigator will make practical recommendations to ensure that the accident does not recur. The recommendations must be agreed with relevant individuals and the actions prioritised. Institute Health and Safety Advisers will track the actions to ensure that they are completed.

3.3 Reporting

The local Health and Safety Committee should meet regularly to discuss and review the findings of all incidents, along with the potential risk if a similar incident were to occur. All serious accidents and/or those with high risk potential should be reported to senior management.

3.4 RIDDOR Incidents

Certain incidents must be notified to the local enforcing authority (HSE) under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) 1995. The prescribed form (F2508) for notifying the HSE of RIDDOR accidents or occurrences can be downloaded directly from the HSE web address The site also gives further information about RIDDOR. BBSRC institutes have appointed a person responsible for reporting RIDDOR incidents, this is usually the Health and Safety Adviser. It is advised that at least one deputy to that person should be appointed to cover absences. A copy of all RIDDOR forms are copied to the HRCSG.

3.5 Injury Benefit

If you are injured or contract a disease during the course of your work, you may qualify for injury benefits under the Research Councils' Pension Scheme. For more details contact your Human Resources Manager. In such circumstances, we may refer you for medical advice.
